If you require any vaccinations relating to foreign travel you need to complete a Travel Risk Assessment form. These forms are available from reception at Clough or Dundrum Surgery or can be downloaded at the bottom of this page.

It is important to complete and return the Travel Risk Assessment form as soon as possible - at least 6 weeks before you travel.

Once you have returned the completed form to Clough or Dundrum Surgery, a practice nurse will contact you to arrnage an appointment to discuss your travel arrangements and what vaccinations are required as vaccines have to be ordered.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
