The Doctors welcome new patients within our practice area. Any patient wishing to register can check if their address is within the catchment area on the practice area map on our website.

Patients transferring GPs within Northern Ireland/GB should complete the following forms:

Patients arriving in Northern Ireland from outside the UK should complete the following forms:

Please return your completed registration form and health questionnaire to the Surgery along with all required documentation so that your registration can be processed.

New Babies

You should register the birth of your baby immediately with the Registrar of Births. You will then be given a yellow form (Form HS 123) which you should complete and hand to the Doctor’s receptionists as soon as possible.
